We need this information so that we can help you if you have any emergency and so we can carry out our duty of care towards you
An emergency could be as simple as a transport breakdown or it might be a medical emergency, though we hope that will not happen

Who to put for your contact

This might be obvious in many cases.
This should be someone we can contact at short notice if possible.
For example, someone at your home.
If the applicant is under 16, we very much prefer that this should be their parent, guardian or carer.

In general, we prefer 'next of kin' (see right)

The phone number would preferably be a land line as mobiles may be switched off.

Who is "Your next of kin"?

This is not a legal opinion, but Wikipedia says that your next of kin is the highest-placed in this list:

your Husband or Wife otherwise
your Son or Daughter otherwise
your Father or Mother otherwise
your Brother or Sister otherwise
your Grandparent otherwise
your Grandchild otherwise
your Uncle or Aunt otherwise
your Nephew or Niece otherwise

It is not clear whether a "partner", legally recognised or not, can act as next of kin. If that is what you want to say, that is what we will accept.
